Indeed, the anti-inflammatory activity could also be a consequence of the presence of rosmarinic acid, whose antioxidant and anti-inflammatory properties have been confirmed in different experimental tests. The healing properties of comfrey are recognized by different traditional medicines.
Among the trials included in the cited review, three have studied the Aeffectiveness of comfrey in cases of sprained ankle. In two of them, a commercial cream with a special extract of S. Officinale was used (an extractant from which a large part of the pyrrolizidine alkaloids are eliminated, leaving less than 35 ppm). The first is a double-blind, multicenter, randomized, placebo-controlled trial, which included 142 patients with a mean age of 31.8 years, with an acute unilateral sprain suffered no more than six hours previously. Most of the patients were men (78%) to whom comfrey cream or placebo was applied for 8 days at a rate of four applications per day. The treatment was effective and far superior to placebo in terms of reducing pain and edema.
Also regarding mobility and overall efficacy, it was highly rated by both physicians and patients, with better effects seen in the verum group than in the placebo group. No adverse effects were observed, therefore, good tolerability.

Dermatological conditions such as atopic dermatitis, acne, hyperpigmentation disorders or psoriasis are highly prevalent in society. For the most severe cases, drugs whose prolonged use causes side effects are usually used. For this reason, the search for topical treatments based on natural ingredients whose adverse effects are minimal is booming. In this sense, the main objective was the development of an in vitro platform called SimDerma for the massive analysis of ingredients and their effects on molecular targets related to skin pathologies. Of the ingredients tested, potentially interesting activities in dermatology were found in cannabidiol (CBD) and strigolactones. On the one hand, CBD stood out for its antioxidant potential, inducing the Bach1/Nrf2 molecular pathway and the expression of genes such as SQSTM1/p62 or HMOX1 that participate in the oxidative stress reduction response. Additionally, CBD treatment promotes epidermal differentiation and proliferation, increasing the thickness of the epidermis. On the other hand, it was found that both strigol and its derivative, compound 14, inhibited the formation of reactive oxygen species and promoted the induction of Nrf2. It should be noted that unlike strigole, compound 14 presented anti-inflammatory activity by inhibiting NF-kB and STAT3. To carry out topical treatments against skin pathologies, combinations based on CBD were made together with other natural ingredients.
